29 CFR 1910.28(b)(15): Guardrails not less than 2 x 4 inches or the equivalent and not less than 36 inches or more than 42 inches high, with a mid-rail, when required, of 1 x 4-inch lumber or equivalent, and toeboards, were not installed at all open sides on all scaffolds more than 10 feet above the ground or floor. Toeboards shall be a minimum of 4 inches in height. Wire mesh shall be installed in accordance with paragraph (a)(17) of this section. On or about February 2, 2016, employees working on a display sign were exposed to falls of approximately 13 feet to the ground below. Scaffold was not equipped with a guard rail system or other form or fall protection.

29 CFR 1910.28(d)(4): Scaffold legs were not set on adjustable bases or plain bases placed on mud sills or other foundations adequate to support the maximum intended load. On or about February 2, 2016, employees working on a scaffold approximately 13 feet above the ground were exposed to being stuck by collapsing scaffold components. Scaffold legs were not set on a firm foundation.
