29 CFR 1915.134(b): Work rests were not kept a distance not to exceed 1/8 inch from the surface of the wheel.   Alaskan Lady, Engine Maintenance Room:  On or about 10 February 2016, and times prior thereto, The Dayton 8" Bench Grinder, Lot No. 130900502, work rest measured approximately 7/8' from the abrasive wheel.  This condition exposed employees to a struck by and or caught in hazard.

29 CFR 1918.62(d)(1): The employer did not adhere to the manufacturer's ratings for natural fiber rope and natural fiber rope slings provided for use aboard ship and/or did not have the manufacturer's ratings available for inspection.        Alaskan Lady, Deck side, offloading operations:  On or about 10 February 2016 and times prior thereto, an unrated, 3/4' in diameter, crab pot line was being used for offloading pallets of frozen fish. Manufacturer's rating was unavailable.  This condition exposed employees to a struck by hazard.

29 CFR 1915.89(q)(1):  The employer did not conduct an audit of the lockout/tags-plus program and procedures currently in use at least annually to ensure that the procedures and the requirements of this section were being followed and to correct any deficiencies.  F/V Alaskan Lady:  On or about 10 February 2016 and times prior thereto, the ship's lockout/tags-plus program did not include an annual audit.
