29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks.     On or about 10 February 2016, and at times prior thereto, employees operating equipment were not protected from the hazards associated with ingoing nip points and point of operation.    1) Within the Mag area, an employee was using a table mounted Baldor Belt Sander to deburr small parts in which a guard was attached but not being utilized during operation, exposing the employee to both amputation and caught in hazards.  2) Near the Frame line 2011, a Roll-In Band Saw, Model EF1459, was being used to cut small metal pieces by hand without the guard being placed near the point of operation, exposing employees to an amputation hazard.

On or about 10 February 2016, and at times prior thereto, electrical equipment and components were being used within the workplace in a manner not in accordance with the listing instructions or labeling, exposing employees to a fire hazard or an electrical hazard of approximately 120 volts. a) Frame Line 2011 - AIMI Station: An extension cord was providing power to a computer station while being plugged into a re-locatable power tap, which was zip tied to a rolling stand. b) Frame Line 2011: Two re-locatable power taps were connected in series to provide power to various hand-held tools. One of those re-locatable power taps was connected to a Tripp-Lite Uninterruptible Power Supply. c) Frame Line 2011: Two extension cords were connected in series to provide power to various equipment while one cord was directly plugged into an outlet. d) Frame Line 2011: Two re-locatable power taps were zip tied to the metal frame of a table in a manner making them permanent wiring instead of temporary wiring. e) Small Parts Area: Two re-locatable power taps were zip tied to the metal frame of a table in a manner making them permanent wiring instead of temporary wiring.

On or about 10 February 2016, and at times prior thereto, spray and squirt bottles found within the facility had no product identifier and words, pictures, symbols, or combination thereof to identify the contents contained, exposing the workers using such bottles to various physical and health hazards.  a) Mag Area: Employees were using an unidentified spray bottle that contained a yellow liquid. The container was said to contain Stamping Oil, ECO Draw HVE1.  b) Gunsmithing Area: Employees were using an unidentified squirt bottle that contained a yellow liquid. The container was said to contain a coolant, Blasocut BC 940.  c) Paint Booth: Employees were using an unidentified spray bottle that contained a clear liquid. The container was said to contain Methyl Ethyl Ketone, MEK, which is a highly flammable liquid and vapor.    STI International, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1900.1200(f)(8), which was contained in OSHA inspection number 314306465, citation number 01, item number 004 and was affirmed as a final order on 07/25/2011, with respect to a workplace located at 114 Halmar Cove, Georgetown, TX 78628.
