29 CFR 1910.178(a)(5): The powered industrial truck was equipped with front-end attachments other than factory installed attachments and the employer did not request that the truck be marked to identify the attachments and show the approximate weight of the truck and attachment combination at maximum elevation with load laterally centered: 480 Housatonic St, Lenox Dale, MA On February 17, 2016 and at times prior, employees operated a Yale powered industrial truck with an Abaco boom front-end attachment installed without having requested approval or received a data plate with updated capacities from forklift manufacturer.
