OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees, in that employees were exposed to crush and/or struck by injuries. a. The Macomb Group, Inc. - Northwood, Ohio: On or about February 17, 2016, the employer did not ensure that a safety latch on the hook of the north five ton Shawbox overhead hoist (underhung on bottom flange) was used with under-the-hook lifting slings. Employees were exposed to crush injuries in that the load and/or lifting devices could become removed from the hook as employees used the hoist and lifting devices to lift parts in the warehouse bay area. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence, or written records.

The Macomb Group, Inc. - Northwood, Ohio: On or about February 17, 2016, employees were exposed to fall hazards of approximately 50 inches when accessing the truck bay to unload/load materials from delivery trucks. The open sided floor around the truck bay was not equipped with guardrails to protect employees from falling to the lower level.
