29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of hazardous energy when employees were engaged in activities such as clearing jams, setting up machines: On or about January 26, 2016: Employees engaged in tool changing activities at the OKK 1000 CNC milling machine in the High Bay were exposed to amputation hazards in that energy control procedures were not utilized prior to physically entering the machine to free a jammed tool.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: Employees engaged in machining activities at the OKK 1000 CNC milling machine in the High Bay were exposed to struck-by and amputation hazards in that interlocks were not provided to ensure the doors remain closed during operation.

29 CFR 1904.39(a)(3): The employer did not report the fatality, in-patient hospitalization, amputation, or loss of an eye within the 24 hour time period by using one of the following methods: (i) By telephone or in person to the OSHA Area Office that is nearest to the site of the incident. (ii) By telephone to the OSHA toll-free central telephone number, 1-800-321-OSHA (1-800-321-6742): On or about January 26, 2016, an injury occurred at the workplace requiring an in-patient hospitalization. This incident was not reported to OSHA until February 11, 2016.
