29 CFR 1915.506(b)(1): The employer did not physically isolate the fixed extinguishing systems or utilize other positive means to prevent the systems discharge before work was done in that space which system was located. A. On or about February 9, 2016, the employer did not ensure that the fixed fire extinguishing system was properly isolated to prevent discharge during a required system check. Employees performing the fire system check did not ensure that all the Halon bottles were disconnected before performing their routine test. Halon was discharged into several areas of the ship requiring evacuation.

29 CFR 1915.506(e)(2): When testing a fixed extinguisher system did not involve a total discharge, the employer did not make sure that the system's extinguishing agent medium was physically isolated and that employees not directly involved in the testing were evacuated from the protected space. A. On or about February 9, 2016, the employer did not ensure that system's extinguishing agent was physically isolated and employees not directly involved with the testing were evacuated from the protected space. Employees performing the fire system check did not ensure that all the Halon bottles were disconnected before performing their routine test. Contractors that were working in the engine area and not involved with the testing were not evacuated from the area where Halon could potentially be discharged.
