On or about February 19, 2016, Production Area: The DoAll Metalmaster band saw, brass tag #423952, had 8 inches of the unused portion of the blade unguarded, exposing employees to laceration and amputation injuries.

On or about February 19, 2016, Production Area: Employees were exposed to amputation and scalping hazards: 1. Bridgeport Milling Machine, S/N J-109253 was missing a change speed belt guard on the left side. 2. Bridgeport Milling Machine, S/N J-82022 was missing a change speed belt guard on the left side. 3. Bridgeport Milling Machine, S/N J-88968 was missing change speed belt guards on both the left and right sides.

29 CFR 1910.1200(e)(1): The employer did not develop, implement, and/or maintain at the workplace a written hazard communication program which describes how the criteria specified in 29 CFR 1910.1200(f), (g), and (h) will be met: A. On or about February 19, 2016, Facility-Wide: The employer had not developed a written hazard communication program that included a list of chemicals used on site and training in the hazards of the chemicals. Employees used a propane-powered TCM forklift, Model FCG15-3L; and Black Beauty abrasive which contained amorphous silica, aluminum oxide, iron oxide calcium oxide, magnesium oxide, titanium oxide and crystalline silica.
