29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   On the roof of a residential building, north side, on or about February 18, 2016, employees without fall protection were exposed to fall hazards greater than 6 feet to the unpaved ground below, while installing shingles on a roof with a pitch of 4:12.

29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift: Note:  As of January 1, 1998, subpart M of this part (1926.502(d)) provides that body belts are not acceptable as part of a personal fall arrest system.  The use of a body belt in a tethering system or in a restraint system is acceptable and is regulated  North Side, Genie Aerial Lift (Model: S-60/ Equipment Rental #61):  On or about February 18, 2016, employees were exposed to fall hazards of approximately 27 feet in height to the unpaved ground below, when installing shingles on the roof of an apartment building from an aerial lift without the use of a personal fall arrest system.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  On the roof of a apartment building, north side, on or about February 18, 2016, employees without fall protection were exposed to a fall hazards of 27 feet to the unpaved ground below, while not being trained on the procedures to recognize fall hazards.
