(a) Monsour Hospital, 30 Lincoln Highway, Jeannette, PA: On or about February 18, 2016, employees were exposed to fall hazards from an unguarded elevator shaft opening on the 9th floor. (b) Monsour Hospital - 30 Lincoln Highway, Jeannette, PA: On or about February 18, 2016, employees working to conduct demolition activities such as but not limited to asbestos abatement were exposed to numerous fall hazards throughout the hospital from the windows being removed
