29 CFR 1910.147(d)(5)(i): All potentially hazardous stored or residual energy was not relieved, disconnected, restrained or otherwise rendered safe after the application of lockout or tagout devices to energy isolating devices: a) Lumber Yard, Husky Grapple Loader: On or about February 17, 2016 and at times prior, the employer exposed employees to crushed-by and amputation hazards in that the crane block and boom were not secured by blocking or other feasible manner to isolate stored mechanical energy (gravity) during maintenance.
