29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:    A lockout/tagout program, including developing and implementing specific lockout procedures, training, and annual inspection was not developed.  Lockout work had occurred including wiring an electrical cord (incorrectly) to the building circuit breaker box and other work, such as, but not limited to, replacing a hydraulic line on the forklift.  (Note: the program and specific procedures for each type of equipment and machines has to be in writing, and training for employees that perform lockout work must be documented.  Training must be provided as described at 1910.147(c)(7)(i)(A)).    Abatement documentation is required for this violation.

29 CFR 1910.303(g)(2)(i): Except as elsewhere required or permitted by Subpart S of Part 1910, live parts of electric equipment operating  at 50 volts or more were not guarded against accidental contact by use of approved cabinets or other forms of approved enclosures or by any of the means identified in paragraphs (A), (B), (C), and (D) of 29 CFR 1910.303(g)(2)(i):    In the office, the building circuit breaker box was not covered exposing employees to live 110/220 V electricity.    Abatement documentation is required for this violation.

29 CFR 1910.303(c)(3)(i): All splices and joints and the free ends of the conductors were not covered with an insulation equivalent to that of the conductors or with an insulating device identified for the purpose.    In the office, the overhead, fluorescent light power cord (two wire) was spliced to a three wire extension cord with electrical tape, therefore resulting in a potential electrical exposure hazard from insufficient insulation.  Abatement documentation is required for this violation.

29 CFR 1910.334(a)(2)(ii): There was a defect or evidence of damage that could have exposed an employee to injury and the defective or damaged item was not removed from service:  In the office, the overhead, fluorescent light power cord (two wire) was spliced to a three wire extension cord.  There was damaged insulation on the cord potentially exposing employees to an electrical shock.   Abatement documentation is required for this violation.
