29 CFR 1910.147(c)(4)(ii): Procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, 29 CFR 1910.147(c)(4)(ii)(A), (c)(4)(ii)(B), (c)(4)(ii)(C) and (c)(4)(ii)(D):  On or about 2/16/2016 in the press production area at 130 Quality Drive, NW in Thomson, GA:  Employees setting and removing molds from the Press were not provided specific written energy control procedures that address all of the energy sources required to be de-energized while performing service and or maintenance.    Press equipment energy sources were not limited to the following: a. Electrical - Max Volts 480 b. Hydraulic  c. Water Pressure - Max Pound Per Square Inch (PSI) 235 d Gravity

29 CFR 1910.212(a)(5): Fan blade guard(s) were not provided where the periphery of the blades was less than seven feet from the floor or working level:  On or about 2/16/2016 in the press production area at 130 Quality Drive in Thomson, GA:  During the inspection, it was determined that the employees operating the cooling equipment were exposed to an amputation hazard from the unguarded rotating fan blades. One employee amputation injuries on 1/22/2016 were caused by the unguarded fan blades on the cooling equipment.

29 CFR 1910.147(c)(6)(i): The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed:  On or about 2/16/2016 in the press production area at 130 Quality Drive, NW in Thomson, GA: The employer did not perform annual periodic inspection on the equipment that the authorized employees were required to perform but not limited to maintenance, repairs, services, setup, adjustments, and cleaning on the machine and equipment.   Cincinnati 220 Injection Molding Press - most recent review of the procedures was 5/6/2014 Several Hydraulic Separation Press 104, 105, 107, 110, 133, 134, and 135 - most recent review of the procedures was 2/5/2014.  The Thomson Plastics, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.147(c)(6)(i), which was contained in OSHA inspection number 315741868, citation number 2, item number 2 and was affirmed as a final order on 7/31/2012, with respect to a workplace located at 130 Quality Drive NW, Thomson, GA.
