29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with paragraph (b) or (c) of this section: a) Jobsite, 3912 Perrysville Avenue, Pittsburgh, PA 15214: Observed on or about February 18, 2016, employees working at the bottom of a trench that was approximately 10 feet deep with vertical sidewalls were not protected from cave-ins. The employees were exposed to an engulfment hazard while engaged in work at the bottom of the excavation.

The competent person did not immediately remove employee(s) from the hazardous area until the necessary precautions could be taken to ensure their safety.

29 CFR 1926.651(i)(3): Sidewalks, pavements, or appurtenant structures had been undermined and a support system or another method was not provided to protect employees from the possible collapse of such structures: a) Jobsite, 3912 Perrysville Avenue, Pittsburgh, PA 15214: Observed on or about February 18, 2016, a sidewalk that had been undermined was not supported to prevent it from collapsing into the excavation. Employees working in areas of the excavation beneath the unsupported sidewalk were exposed to struck-by and crushed-by hazards.
