On or about February 17, 2016, on the southwest side of the roof of the pavilion, employees installing 1 x 2s to the roof were using pneumatic nail guns and did not wear eye protection, exposing themselves to the hazard of being struck in the eye or face by flying debris.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system: a. On or about February 17, 2016, on the southwest side of a pavilion that is connected to a residential home, an employee installing concrete tiles to the roof was not protected from a 13 feet fall hazard by the use of a personal fall arrest system. b. On or about February 17, 2016, on the north side of a residential home, an employee installing concrete tiles to a low slope roof was not protected from a 23 feet 8 inch fall hazard by the use of a personal fall arrest system. Cartercraft Roofing, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(11), which was contained in OSHA inspection number 315479063, citation number 01, item number 01 and was affirmed as a final order on September 21, 2011, with respect to a workplace located at 2306 Hwy 77, Panama City, FL 32402. Cartercraft Roofing, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(11), which was contained in OSHA inspection number 1061918, citation number 02, item number 01 and was affirmed as a final order on September 4, 2015, with respect to a workplace located at 111 Cashel Mara Drive, Southport, FL.
