29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift:     (Note:  As of January 1, 1998, subpart M of this part (1926.502(d)) provides that body belts are not acceptable as part of a personal fall arrest system.  The use of a body belt in a tethering system or in a restraint system is acceptable and is regulated under 1926.502(e).)     (a) North East Side of Building: On or about February 17, 2016 the employer did not ensure that an employee working from a Genie Z-60/34 aerial lift was protected from a fall/throw hazard of approximately 20 feet while performing metal roof installation work.

29 CFR 1926.502(d)(8): Horizontal lifelines were not designed, installed, and used under the supervision of a qualified person, as part of a complete personal fall arrest system, which maintained a safety factor of at least two:    (a) North East Side of Building: On or about February 17, 2016 employees installing sheet metal roofing placed a cable rope around the building cupola as a horizontal life line and to serve as the anchor point of the personal fall arrest system.  The fall arrest system was not installed by a qualified person and the employer did not provided information that the cable rope system maintained a safety factor of two (2), exposing employees to a fall hazard of 20 feet.

29 CFR 1926.503(a)(2)(iii): The employer did not assure that each employee has been trained, as necessary, by a competent person in the use and operation of guardrail systems, safety net systems, warning line systems, safety monitoring systems, controlled access zones, and other protection to be used.  (a) The employer did not ensure that employees exposed to fall hazards had been trained to recognize and minimize the fall hazards with their work and to employ fall protection systems and methods to prevent falls.
