Within the facility, on or about 2/19/16:  Lockout/tagout procedures were not developed nor utilized during maintenance and/or servicing on equipment such as but not limited to air compressors and earth moving equipment to prevent the unexpected energizing, start up or release of stored energy of the equipment.  Hazardous energy includes but is not limited to electrical, pneumatic and gravitational.    Abatement certification must be submitted for this item.

29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s):    a.  In the maintenance shop, on or about2/19/16:  A hydraulic press having two-hand controls was not guarded in that the controls were not protected against accidental contact due to their proximity to each other.  The two-hand controls could be operated by one hand.    b.  In the maintenance shop, on or about 2/19/16:  Two hydraulic presses having single controls which allows the free hand to come in contact with the point of operation.    Abatement certification must be submitted for these items.

29 CFR 1910.215(b)(9): The distance between the abrasive wheel periphery(s) and the adjustable tongue or the end of the safety guard peripheral member at the top exceeded one fourth inch:    a.  In the welding shop, on or about 2/19/16:  A pedestal grinder was being operated without an adjustable tongue guard being in place.  The distance between the abrasive wheel periphery and the top was one inch.    Abatement certification must be submitted for this item.
