OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to ejection and crushing hazards from operating powered industrial trucks:   a) Production Area:  Employees were exposed to ejection and crushing hazards while operating the King propane forklift which was not equipped with an operator restraint device (seatbelt) as observed on or about February 19, 2016.  b) Production Area:  Employees were exposed to ejection and crushing hazards while operating the World WFG 50 propane forklift which was not equipped with an operator restraint device (seatbelt) as observed on or about February 19, 2016. 29 CFR 1910.1200(h)(3)(iv): Employee training did not include the details of the hazard communication program developed by the employer, including an explanation of the labels received on shipped containers and the workplace labeling system used by their employer; the safety data sheet, including the order of information and how employees can obtain and use the appropriate hazard information:   a) At the facility: Employees worked with hazardous chemicals, including, but not limited to chemicals found in paints such as XAY 9228HS MDI Yellow and X-9016 Blue UV Guard, and the employer did not provide these employees with the required training on the new label elements and safety data sheet format as required by the revised OSHA Hazard Communication Standard incorporating the Globally Harmonized System of Classification and Labeling of Chemicals prior to the effective date of December 1, 2013, as observed on or about February 19, 2016.
