29 CFR 1910.141(d)(2)(i): Lavatories were not made available to employees in the workplace:      (a) Frisco Pallet, LLC @ 6100 Huron Street, Denver, CO 80221:  On or before February 3, 2016, as part of injury/accident prevention responsibilities under it safety program, the employer did not ensure that lavatories were made available to the employees working at this the facility. The sink inside the bathroom had been broken without having hot and cold running water. The bathroom facility was not maintained in a sanitary condition. This conditions exposed employees to health and environmental hazards.      Abatement Note:  Each lavatory shall be provided with:        (1)     Each lavatory shall be provided with hot and cold running water, or tepid running water.      (2)     Hand soap or similar cleansing agents shall be provided.      (3)     Individual hand towels or sections thereof, of cloth or paper, warm air blowers or clean individual sections of continuous cloth toweling, convenient to the lavatories, shall be provided.

29 CFR 1910.178(l)(1)(i): Operators were not trained in the safe operation of powered industrial trucks:      (a) Frisco Pallet, LLC @ 6100 Huron Street, Denver, CO 80221:  On or before February 3, 2016, as part of injury/accident prevention responsibilities under it safety program, the employer did not ensure that each employee who was allowed to operate the two forklifts in the facility were successfully trained as required under the OSHA standard. This condition exposed the employees to the hazard of improper forklift operation.      Abatement Note:  Training shall consist of a combination of formal instruction (e.g., lecture, discussion, interactive computer learning, video tape, written material), practical training (demonstrations performed by the trainer and practical exercises performed by the trainee), and evaluation of the operator's performance in the workplace.

29 CFR 1910.178(q)(1):  Powered industrial trucks with defects or in any way unsafe had not been withdrawn from service until restored to safe operating conditions:    (a) Frisco Pallet, LLC @ 6100 Huron Street, Denver, CO 80221:  On or before February 3, 2016, as part of injury/accident prevention responsibilities under it safety program, the employer did not ensure that the Toyota Forklift, Serial No.17475 was maintained in a safe operating condition and removed from service when the unsafe conditions were identified.  The brakes were defective and the horn was inoperable.  These conditions exposed the employees to the hazards of and unsafe forklift operation.

29 CFR 1910.213(h)(1):  The sides of the lower exposed portion of the blade of radial saw were not guarded to the full diameter of the blade by a device that automatically adjusted itself to the thickness of the stock and remained in contact with the material being cut:      (a)  Frisco Pallet, LLC @ 6100 Huron Street, Denver, CO 80221:  On or before February 3, 2016, as part of injury/accident prevention responsibilities under it safety program, the employer did not ensure that the Wilson Radial Arm Saw, Serial No. 5A 2355 was properly guarded.  The upper hood did not completely enclose the upper portion of the blade down to a point that included the end of the saw arbor.  The sides of the lower exposed portion of the blade were not guarded to the full diameter of the blade by a device that automatically adjusted itself to the thickness of the stock and remained in contact with the material being cut.  This condition exposed the employees to an amputation hazard.

This condition exposed the employees to the hazard of amputation.

Employees worked with saws and wood products, and were exposed to first aid type of injuries.
