29 CFR 1910.212(a)(5): Fan blade guard(s) were not provided where the periphery of the blades was less than seven feet from the floor or working level: James Hardie Building Products, Inc. establishment at 809 S. Woodrow Wilson St., Plant City, FL 33563, as noted on or about 02/18/2016, employees were exposed to amputation hazard as a result of being in the direct proximity of a unguarded blower: a) A Daybin was in operation without rotating parts being guarded, employee right index finger and middle finger were amputated when he tripped and had his hand fall into the blower on or about 02/12/2016.
