29 CFR 1910.132(d)(1): The employer shall assess the workplace to determine if hazards are present, or are likely to be present, which necessitate the use of personal protective equipment (PPE). If such hazards are present, or likely to be present, the employer shall:  29 CFR 1910.132(d)(1)(i): Select, and have each affected employee use, the types of PPE that will protect the affected employee from the hazards identified in the hazard assessment;  a.  In the warehouse where broken bags of chemicals were handled and placed on pallets to be shipped out in tractor trailers, the employer did not perform a personal protection equipment assessment thereby exposing employees to a contact hazard.

29 CFR 1910.178(m)(7): Brakes shall be set and wheel blocks shall be in place to prevent movement of trucks, trailers, or railroad cars while loading or unloading. Fixed jacks may be necessary to support a semitrailer during loading or unloading when the trailer is not coupled to a tractor. The flooring of trucks, trailers, and railroad cars shall be checked for breaks and weakness before they are driven onto.  a.  At the warehouse where a powered industrial truck driver was loading a pallet of chemical fertilizer into the trailer, the truck driver pulled forward and the forklift operator fell off the dock with the forklift thereby injuring the employee.
