Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system:    (a)  On or about February 19, 2016, employees were exposed to injuries from falls, while conducting re-roofing activities, without the benefit of fall protection systems being in place, on a roof in excess of six feet above the lower surface.      Lianro Metal Roofs, Inc. was previously cited for a violation of this Occupational Safety and Health standard or its equivalent standard which was contained in OSHA Inspection Number 730426, Citation Number 1, Item Number 1, and was affirmed as a final order on December 6, 2012, with respect to a workplace located at 2505 Rimrock Drive, Colorado Springs, CO.
