29 CFR 1910.147(f)(1): Testing or positioning of machines, equipment or components thereof. In situations in which lockout or tagout devices must be temporarily removed from the energy isolating device and the machine or equipment energized to test or position the machine, equipment or component thereof, the following sequence of actions shall be followed: Employee performing testing or positioning of machines, equipment did not perform the following: deenergize all systems and reapply energy control procedures in accordance with paragraph (d) to continue servicing and maintenance. On or about February 14, 2016, in the Production Area, Core Build 2, a general maintenance employee was replacing a sprocket and tightening the tension on the chain on the Hytrol Conveyor, E-44, when the following sequence was not followed prior to completion of the maintenance activity.
