29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by rotating parts:  On or about 2/22/2016, at the cotton candy machines 1 through 6 - employees are suffering laceration and burn injuries when being struck-by the leather straps on spinning heads rotating at 3,450 RPM while:  a. #1 cotton candy machine, reaching into the bowls to collect the cotton candy, and       while reaching into the bowls to fill the heads with sugar   b. #2 cotton candy machine, reaching into the bowls to collect the cotton candy, and       while reaching into the bowls to fill the heads with sugar   c. #3 cotton candy machine, reaching into the bowls to collect the cotton candy, and       while reaching into the bowls to fill the heads with sugar   d. #4 cotton candy machine, reaching into the bowls to collect the cotton candy, and       while reaching into the bowls to fill the heads with sugar   e. #5 cotton candy machine, reaching into the bowls to collect the cotton candy, and       while reaching into the bowls to fill the heads with sugar    f. #6 cotton candy machine reaching into the bowls to collect the cotton candy, and       while reaching into the bowls to fill the heads with sugar .

Safety of equipment was not determined using the following considerations: Other factors that contribute to the practical safeguarding of persons using or likely to come in contact with the equipment.  On or about 2/22/2016 at the #1 popcorn kettle; employees performing popcorn popping and packaging operations are exposed to potential electrical shock and burn hazards where the 120/208 V rocker switch was removed exposing the two (2) metal connectors inside.
