29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system. a.) 2829 Kanawha Blvd. East, Charleston, WV: On or about 02/22/16, employees were observed working on a 3/12 sloped roof approximately 12'6" above the ground without the use of fall protection. Shield Roofing and Construction, LLC., was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(11), which was contained in OSHA inspection number 1062520, Citation 01, Item number 001a and was affirmed as a final order on 06/24/15, with respect to a workplace located at 405 Pennsylvania Avenue, Charleston, WV 25302.
