29 CFR 1910.23(c)(1): Every open-sided floor or platform 4 feet or more above adjacent floor or ground level shall be guarded by a standard railing (or the equivalent as specified in paragraph (e)(3) of this section) on all open sides except where there is entrance to a ramp, stairway, or fixed ladder. The railing shall be provided with a toeboard wherever, beneath the open sides, a. Storage Area above the Lunch Area - On or about February 23, 2015, the storage area located approximately seven (7) feet six (6) inches above the floor was not guarded by a complete standard railing, thereby employees were exposed to falls hazards.

29 CFR 1910.23(d)(1)(iii): On stairways less than 44 inches wide having both sides open, one stair railing on each side. a. First floor to Storage Area on Second Level - On or about February 23, 2016, the stairway from the first floor to the storage area was not equipped with a standard railing on each side. Employees were exposed to falls when storing materials around the flight of stairs.

29 CFR 1910.37(b)(4): If the direction of travel to the exit or exit discharge is not immediately apparent, signs must be posted along the exit access indicating the direction of travel to the nearest exit and exit discharge. Additionally, the line-of-sight to an exit sign must clearly be visible at all times. a. Coalition in Support Ecclesia Day/Back Area - On or about February 26, 2016, the employer had several paths of travel to exit doors among the office's rooms that did not have a sign clearly reading "to an exit". These were the following doors; 1. South East - hallway's door. 2. North West - Main Office' s door. 3. North East - warehouse room's door.
