29 CFR 1926.451(b)(5)(ii): The end of a platform greater than 10 feet in length extended over its support more than 18 inches and the platform was not designed and installed so that the cantilevered portion of the platform is able to support employees without tipping, or has guardrails which block employee access to the cantilevered end:     (a) Jobsite, 1201 William Flynn Highway, Glenshaw, PA 15116: On or before February 22, 2016, employees were working unprotected and exposed to fall hazards of approximately 14 feet in that the scaffold stage extended beyond its left support by approximately 6 feet.

29 CFR 1926.452(j)(2): Poles were not secured to the structure by rigid triangular bracing or equivalent at the bottom, top, and other points as necessary:   (a) Jobsite, 1201 William Flynn Highway, Glenshaw, PA 15116: On or before February 22, 2016, employees were working unprotected and exposed to fall hazards of approximately 14 feet in that the scaffold poles were not rigidly supported at its base section.
