29 CFR 1910.147(c)(4)(ii)(B): The energy control procedures did not clearly and specifically outline the steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy.  At the facility, the employer did not have written lockout/tag-out procedures for each type of machine and did not provide written procedures for shutting down, isolating, blocking, and securing machines to control each hazardous energy source, exposing employees to hazards such as amputations and electric shock.

29 CFR 1910.147(c)(6)(ii): The employer shall certify that the periodic inspections have been performed. The certification shall identify the machine or equipment on which the energy control procedure was being utilized, the date of the inspection, the employees included in the inspection, and the person performing the inspection.  At the facility, the employer lockout/tag-out program review did not identify the machines on which the specific procedure was being utilized nor the employees included in the inspection, exposing employees to hazards such as amputations and electrical shock.

29 CFR 1910.147(c)(7)(i): The training program of the employer did not include all the training elements listed under Items A-C of 29 CFR 1910.147(c)(7)(i).  Throughout the facility, the training program for lockout/tagout procedures did not contain all the required elements for training, including, but not limited to, the following:      a)  For the authorized employee, training did not adequately instruct them on the recognition of applicable hazardous energy sources, type and magnitude of the energy available in the workplace, and methods and means necessary for energy isolation and control.      b)  For the affected employees, training did not adequately instruct them in the purpose and use of energy control procedures.
