29 CFR 1910.132(a):  Protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, shall be provided, used, and maintained in a sanitary and reliable condition wherever it is necessary by reason of hazards of processes or environment, chemical hazards, radiological hazards, or mechanical irritants encountered in a manner capable of causing injury or impairment in the function of any part of the body through absorption, inhalation or physical contact:    On or about 24 February 2016, employees were exposed to falls in excess of six (6) feet while required to climb onto storage racks without fall protection equipment to retrieve stored materials. This exposed employees to a fall hazard.    Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1910.151(c): Where the eyes or body of any person may be exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body shall be provided within the work area for immediate emergency use:    On or about 24 February 2016, employees were exposed to chemical exposure hazards such as severe skin burns and eye damage as a result of the employer's failure to ensure that suitable facilities for quick drenching or flushing of the eyes were readily available in the work area. Employees were exposed to eye injury from battery servicing/charging and transfer of hazardous liquid chemicals whithout providing adequate eyewash facilities.    Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1910.178(l)(1)(ii): Prior to permitting an employee to operate a powered industrial truck (except for training purposes), the employer shall ensure that each operator has successfully completed the training required by this paragraph (l), except as permitted by paragraph (l)(5).    On or about 24 February 2016, the employer did not ensure that each operator had successfully completed the training consisting of a combination of formal instruction (e.g., lecture, discussion, interactive computer learning, video tape, written material), practical training (demonstrations performed by the trainer and practical exercises performed by the trainee), and evaluation of the operator's performance in the workplace. This exposed employees to a struck by hazard.    Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1910.1200(e)(1): Employers shall develop, implement, and maintain at each workplace, a written hazard communication program which at least describes how the criteria specified in paragraphs (f), (g), and (h) of this section for labels and other forms of warning, safety data sheets, and employee information and training will be met, and which also includes the following:    On or about 24 February 2016, the employer did not develop, implement, and maintain at the workplace a written hazard communication program which describes how the criteria specified in 1910.1200(f), (g), and (h) will be met for employees exposed to chemicals, including gasoline, brake fluid, diesel fuel, motor oil, and cleaning chemicals.    Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
