29 CFR 1910.23(a)(8): Every floor hole into which persons can accidentally walk were not guarded:  a) On or about 2/23/2016, on the south side of the second floor of the mill, the employer allowed employees to conduct housekeeping, daily rounds, and maintenance and did not ensure that a hole was guarded.    b) On or about 2/23/2016, in the rice bin area, the employer allowed employees to check on equipment and empty rice bins without ensuring that holes through the grating were guarded.
