OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to struck-by hazards in the event of the galvanizing bucket separating from the lifting device.    In the coating area, the employer did not ensure hooks used as lifting devices had safety latches covering the throat openings, exposing employees to struck-by hazards in the event of equipment failure;    a) throughout the facility, hoist chain hooks were used that did not have safety latches installed;  b) in the coating area, the hook on the chain hoist to lift the spinner was missing the safety latch; and   c) in the coating area, the spinner hook did not have a safety latch.

In the shipping department, employees operated a Skilsaw circular saw with a cut and spliced cord and used an extension cord that was spliced and hard wired into the gang box, exposing employees to electrical hazards.
