29 CFR 1910.178(m) (5) (ii): A powered industrial truck shall not be left unattended when the operator is 25 ft. or more away from the vehicle which remains in view, or whenever the operator leaves the vehicle and it is not in view:  a.  4220 W White Road, Oakwood, GA (Shop Area) - Employees were exposed to struck-by hazard.   An employee left the powered industrial truck running and unattended with key inside the ignition and emergency brakes not set, while the employee went to the break room, exposing surrounding employees to the danger of a struck-by of an unintended movement of the truck, on or about 02/23/16.

Employees use the mobilize rollers for transporting the trusses to areas throughout the shop facility.  The pulley on the drive mechanism was not guarded, thus exposing employees to hazards of in-running nip points, on or about 02/23/16.

The belt was not fully guarded, thus exposing employees to a potential hazards of in-running nip points and/or injury resulting from belt breakage, on or about 02/23/16.

29 CFR 1910.303(b)(1)(iv): Electric equipment was not free from recognized hazards that were likely to cause serious physical harm to employees, such as damaged electrical insulation:  a.  4220 W White Road, Oakwood, GA (Shop Area) - Employees were exposed to an electrical hazard. The electrical power cord to the Circular DeWalt "Chop" Saw, which employees use on a daily basis, was damaged to the outer covering and inner insulation, exposing the bare live wires, on or about 02/23/16.

29 CFR 1904.39(a): The employer did not report the in-patient hospitalization, amputation, or loss of an eye to OSHA within twenty-four (24) hours after the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident:    a. 4220 W White Road, Oakwood, GA  Retreat Center) - The employer failed to notify the Atlanta-East OSHA office of an in-patient hospitalization/amputation within 24 hours of being admitted.  The employee was admitted as an in-patient on February 10, 2016 and was not reported to the OSHA Area Office until February 17, 2015.
