29 CFR 1926.451(c)(2)(ii): Unstable objects were used to support scaffolds or platform units:  Worksite, 129 Littleton Avenue, Newark, NJ  a)  Employees were exposed to fall hazards of 40 ft. when performing masonry construction from an unstable scaffold plank platform that was leveled by using cinderblock bricks.  b)  Employees were exposed to fall hazards of 40 ft. when performing masonry construction from an unstable scaffold plank platform that was inadequately positioned with lumber wedged between the tubular welded frame and said plank.  Violation occurred on or about 2-23-16

29 CFR 1926.451(g)(1)(vii): Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section:  a) Worksite, 163 Littleton Avenue, Newark, NJ  An employee performing cinderblock bricklaying from a tubular welded scaffold frame platform approximately 40 feet above grade was not protected by any means of fall protection.   Violation occurred on or around 2-23-16
