Specifically the employer is failing to issue bin entry permits for employees entering the grain bins as part of their maintenance program or the cleaning of the bins in preparation for new product.  Hazards include but are not limited to:           i.        Employers failure to issue a permit exposes the employees to engulfment hazards when the employer has not reviewed the permit to ensure the grain is not greater than kneed deep or has overhead bridging conditions.          ii.        Employers failure to issue a permit exposes the employees to respiratory hazards when the employer has not reviewed the permit to verify the air quality testing has been accomplished.       iii.        Employers failure to issue a permit exposes the employees to emergency escape and/or recovery hazards when the employer has not reviewed the permit to ensure rescue procedures and equipment are used.       iv.        Employers failure to issue a permit exposes the employees to emergency escape and/or recovery hazards when the employer has not reviewed the permit to ensure that an attendant is assigned and present during entry.        v.        Employers failure to issue a permit exposes the employees to caught between, amputation and engulfment hazards when the employer has not reviewed the permit to ensure that all mechanical and electrical equipment is de-energized and Locked out prior to entry.
