29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6): On or about November 24, 2015, the employer failed to ensure employees cleaning the Glass Roll Press model number MHM03500416, serial number 10253, performed all steps of lockout/tagout. An employee's arm was caught in the roller assembly when another employee turned power back on: Employees were exposed to machine hazards associated with electrical and pressurized pneumatic energy. The employer failed to implement energy control application steps as the machine was not shut down or turned off to perform the servicing work. As a result, the remaining applicable energy control elements, involving lockout/tagout device application [(d)(4)], dissipation of residual energy [(d)(5)(I)], and verification of isolation [(d)(6)] were not implemented to protect employees from machine servicing hazards. The Vista Window Company, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.147(d)(4)(i), which was contained in OSHA inspection number 1003030, citation number 01, item number 006 and was affirmed as a final order on May 7, 2015, with respect to a workplace located at 1701 Henn Parkway SW, Warren, Ohio 44481.
