29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips:  15 Railroad Drive, Northborough, MA: On or about February 23, 2016 employees were observed using downward ram balers which had broken interlock devices allowing such equipment to be operated with the top door hatch in the open position while the ram was in its downward stroke exposing employees to crushing and amputation hazards.  (a) SELCO Baler, #V5-HD. Downward ram baler which employees were using with door open while ram was operating due to inoperative safety interlocks. (b)International Press and Shear Baler serial # 109278.  Downward ram baler which employees were using with door open while ram was operating due to inoperative safety interlocks.  Specific abatement documentation that this violation has been corrected must be provided within 10 days of the abatement in accordance with 29 CFR 1903.19(d)(1)

29 CFR 1910.303(b)(7)(i): Unused openings in boxes, raceways, auxillary gutters, cabinets, equipment cases, or housings were not effectively closed to afford protection substanially equivalent to the wall of the equipment:   Panel, Power, 15 Railroad Drive, Northborough, MA: On or about February 23, 2016 employees were working at or near a power panel which had an opening on its dead front that was not effectively closed exposing employees to live electrical parts.
