29 CFR 1926.453(b)(2)(v): A body belt with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift: Worksite located at 10914 Hefner Pointe Drive. Oklahoma City, OK 73120. On or about February 22, 2016 an employee was performing waterproofing work from an aerial lift without the use of a fall protection harness and lanyard. The employee was exposed to a fall hazard of more than 6 feet.
