29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents.     On or about February 23, 2016 an employee was not protected by safety glasses while operating a pneumatic nail gun, exposing the employee to a potential eye injury.     Abatement certification is required of this item in accordance with the requirements of 29 CFR 1903.19(c).

29 CFR 1926.1053(b)(1): Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder.         On or about February 23, 2016 an employee used a portable ladder to gain access to a scaffold platform and the ladder did not extend at least three feet past the platform, exposing the employee to a fall.        Abatement certification is required of this item in accordance with the requirements of 29 CFR 1903.19(c).

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):       On or about February 23, 2016 employees were not protected from a fall while performing residential construction activities.      a) An employee was not protected from a fall while sheeting a residential roof, exposing the employee to a 13 foot fall.      b) Employees were not protected from a fall while working inside of a residential property where the basement access was not guarded by a complete guardrail, exposing employees to a 11.4 foot fall.        Sabala Construction, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(1), which was contained in OSHA inspection number 704838, citation number 1, item number 2a and was affirmed as a final order on December 12, 2012, with respect to a workplace located at 3235 Norman Ave. Joliet, IL 60431.    Abatement documentation is required of this item in accordance with the requirements of 29 CFR 1903.19(d).
