29 CFR 1926.502(d)(5): Snaphooks shall be sized to be compatible with the member to which they are connected to prevent unintentional disengagement of the snaphook by depression of the snaphook keeper by the connected member, or shall be a locking type snaphook designed and used to prevent disengagement of the snaphook by the contact of the snaphook keeper by the connected member. Effective January 1, 1998, only locking type snaphooks shall be used.  Located at the Lever Park Water Tower Project employee(s) performing containment tarp removal associated operations were observed working / standing on the tarp 5/16 horizontal suspension or similar cable(s) / outrigger(s) system with their personal fall arrest lanyard snaphook connector rigged to same or similar cables which was not sized to be compatible with the cable member to which they are connected to prevent unintentional disengagement during a fall arrest condition of the snaphook by depression of the snaphook keeper against the connected cable member or connected cable member and affiliated outrigger eye suspending the cable exposing employees to a fall potential in excess of 125 feet.

29 CFR 1926.502(d)(16)(iii): Personal fall arrest systems, hen stopping a fall, shall be rigged such that an employee can neither free fall more than 6 feet (1.8 m), nor contact any lower level;  Located at the Lever Park Water Tower Project employee(s) performing containment tarp removal associated operations were observed working / standing on the tarp suspension or similar cable(s) / outrigger(s) system with their personal fall arrest lanyard snaphook connector rigged to same or similar cables at foot level such that employees free fall would be in excess of 10 feet prior to shock absorber engagement and the fall beginning stop.

29 CFR 1926.501(b)(15): "Walking/working surfaces not otherwise addressed." Except as provided in 1926.500(a)(2) or in 1926.501 (b)(1) through (b)(14), each employee on a walking/working surface 6 feet (1.8 m) or more above lower levels shall be protected from falling by a guardrail system, safety net system, or personal fall arrest system.  Located at the Lever Park Water Tower Project an employee performing containment tarp removal associated operations was observed working / accessing on the tarp  horizontal 5/16 suspension or similar cable(s) /  outrigger(s) system with single personal fall arrest lanyard / snaphook system connector rigged to the horizontal 5/16 cable, at each perimeter exterior outrigger beam disengage the snaphook reaming unprotected until reconnection to the horizontal cable could be made on the opposite side of each perimeter exterior outrigger the employee passed to access a new work location exposing the employee a total fall potential in excess of 125 feet.
