29 CFR 1926.451(g)(4)(i): Guardrail systems were not installed along open sides and ends of platforms: a) Worksite - on or about February 23, 2016, CSHO observed on employee working on a pump-jack scaffolding, approximately 23 feet above the ground, the scaffolding has open sides, with no end guardrails, creating a potential for the employee to fall.
