29 CFR 1910.253(b)(4)(iii): Oxygen cylinders in storage were not separated from fuel-gas cylinders or combustible materials (especially oil or grease), a minimum distance of 20 feet (6.1 m) or by a noncombustible barrier at least 5 feet (1.5 m) high having a fire-resistance rating of at least one-half hour: a) Tri-Vet Design and Fabrication: Employees worked inside the job shop where oxygen cylinders and fuel-gas cylinders (i.e., acetylene and propane) were not separated by a minimum distance of 20 feet or by a barrier having a fire-resistance rating of at least one-half hour, discovered on or about 02/23/2016. ABATEMENT CERTIFICATION REQUIRED

29 CFR 1910.1200(h)(3)(iv): Employee training did not include the details of the hazard communication program developed by the employer, including an explanation of the labels received on shipped containers and the workplace labeling system used by their employer; the safety data sheet, including the order of information and how employees can obtain and use the appropriate hazard information: a) Tri-Vet Design and Fabrication: Employees worked with hazardous chemicals, including, but not limited to, CRC Brakleen, and the employer did not provide these employees with the required OSHA Globally Harmonized System of Classification and Labelling of Chemicals (GHS) training prior to the effective date of December 1, 2013, discovered on or about 02/23/2016. ABATEMENT CERTIFICATION REQUIRED
