29 CFR 1926.502(b)(13): When guardrail systems were used around holes which were used as access (such as ladderways), they were not provided with a gate, or were not so offset that a person could not walk directly into the hole.    a) Jobsite: Stairway floor hole, 4th floor, east of hoist - Subcontractor employees were exposed to an approximate 9 foot fall hazard during construction activities of the 16 story apartment tower as the guardrail system around the floor hole did not prevent the employees from directly walking into the floor hole.    Observed on or about 2/24/16.
