29 CFR 1910.303(b)(1)(iv): Electric equipment shall be free from recognized hazards that are likely to cause death or serious physical harm to employees, based on the lack of electrical insulation:    On or about February 24, 2016, the C Frame rivet press used in the Front Wall Area had electrical terminals on the controls covered in electrical tape that did not provide an insulating value.

29 CFR 1910.305(g)(2)(iii): Flexible cords and cables were not connected to devices and fittings so that strain relief was provided that would prevent pull from being directly transmitted to joints or terminal screws:  On or about February 24, 2016, the flexible cords on the C Frame rivet press used in the Front Wall Area were not connected to the press controls so that strain relief was provided.

29 CFR 1910.212(a)(3)(ii): The point of operation of machines whose operation exposes an employee to injury, was not guarded:    On or about January 28, 2016, the point of operation of the C Frame rivet press used in the Front Wall Area was not guarded in that the controls did not incorporate a safety function and were not designed and constructed so as to protect against inadvertent operation of the press.    Stoughton Trailers, LLC was previously cited for a violation of this Occupational Safety and Health Standard [29 CFR 1910.212 (a)(3)(ii)], which was contained in OSHA inspection number 974394, citation number 2, item number 1 and was affirmed as final order on August 11, 2014, with respect to a workplace located at 1111 Veterans Blvd in Stoughton, WI.
