29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    (a) San Fab Conveyor on or about February 23, 2016 at the address of 2000 Superior St. Sandusky, OH: The employer failed to ensure the incoming nip-points between the drive chains and sprockets contained on the chain driven line roller conveyor was adequately guarded during function test operations.  Employees performing these test operations in an unguarded condition were exposed to amputation hazards at the incoming nip-points.

29 CFR 1910.305(g)(2)(iii): Flexible cords were not connected to devices and fittings so that tension would not be transmitted to joints or terminal screws:    (a) San Fab Conveyor on or about February 23, 2016, at the address of 2000 Superior St. Sandusky, OH: The employer failed to ensure that the flexible power cord providing temporary power to a conveyor was spliced or joined in a manner that effectively secures the temporary connection; preventing incidental employee exposure to live wires. The open splice, joined by wire nuts was not protected by an enclosure and appropriate fittings. Employees working around the 480 volt spliced cord were exposed to potential electrical shock hazards.
