29 CFR 1910.95(g)(6): At least annually after obtaining the baseline audiogram, the employer did not obtain a new audiogram for each employee exposed at or above an 8-hour time-weighted average of 85 decibels:    a)  In the Blasting Building on April 8, 2016, where an employee performing wet blasting activities had not received an annual audiogram.  The employee was exposed to 553.0% of the allowable 8-hour noise dose.  The equivalent noise level of 553.0% is approximately 102.336 dBA.  Sampling was performed for 313 minutes.  Zero exposure was assumed for the remaining  minutes of the work shift.    b)  In the Batch Building on April 8, 2016, where an employee performing powder coating activities in the spray booth had not received an annual audiogram.  The employee was exposed to 83.60% of the allowable 8-hour noise dose.  The equivalent noise level of 83.60% is approximately 88.7 dBA.  Sampling was performed for 434 minutes.  Zero exposure was assumed for the remaining 46 minutes of the work shift.
