29 CFR 1910.23(e)(1): Standard railing(s) did not consist of top rail, intermediate rail and/or posts and/or did not have a vertical height of 42 inches (106.7 cm) nominal from upper surface of top rail to floor, platform runway, or ramp level:    a. On or about 05/09/16, at the warehouse - employees cleaning and clearing the loft were exposed to a 8 foot fall hazard, in that, the intermediate guard rail was not installed and the top guard rail had 33 inches of vertical height.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):     a. On or about 02/22/2016, at the job site - an employee installing gutters from the top of the first story roof was exposed to an 8 foot fall hazard, in that, fall protection was not used. The employee was injured after falling from the roof.     THE BUILDER SERVICE GROUP, INC. D.B.A. GUTTER KING WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD OR ITS EQUIVALENT STANDARD 29 CFR 1926.501(B)(13), WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 476399, CITATION NUMBER 01, ITEM NUMBER 001 AND WAS AFFIRMED AS A FINAL ORDER ON 09/11/2012, WITH RESPECT TO A WORKPLACE LOCATED AT 10006 HIDDEN RIVER DRIVE, ORLANDO, FL 32801.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:     a. On or about 02/22/2016, at the job site - the ladder that was being used by an employee to gain access to a roof landing was not positioned the minimum 3 feet above the eave or landing of the roof to provide the employee safe access and egress exposing him to an 8 foot fall hazard. The ladder was approximately 18 inch above the eave or landing area.    THE BUILDER SERVICE GROUP, INC.
