29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury:       The side of the residential home under construction, south: On or about February 23, 2016, and at times prior thereto, employees were exposed to a fall hazard greater than 6 feet while installing drip edge metal trim on the outside of a residential home without being trained on fall hazard recognition.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:       The side of the residential home under construction, west: On or about February 23, 2016, and at times prior thereto, employees were exposed to a fall hazard greater than 6 feet, while using a 16 foot extension ladder to access the building roof that had the side rails approximately 13 inches beyond the landing, to install drip edge metal trim on the outside of a residential home.
