29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents. On or about 22 February 2016, at this residential construction site, employees were exposed to struck by hazards when metal rods were cut with STIHL Cutquik machine created sparks, without proper eye and face protection.
