The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to combustible carbon dust deflagration, explosion, or other fire hazards while working at or near a dust collector which was not adequately located or designed to prevent or minimize employee exposure: a) At the TEDA Area, on or about February 25, 2016 - The Mikro-Pulsaire Dry Dust collector was located inside of the building and was not equipped with explosion protection, such as but not limited to, explosion venting or deflagration suppression systems.
