29 CFR 1926.760(b)(3): Be provided, at heights over 15 and up to 30 feet above a lower level, with a personal fall arrest system, positioning device system or fall restraint system and wear the equipment necessary to be able to be tied off; or be provided with other means of protection from fall hazards in accordance with paragraph (a)(1) of this section. a) Worksite - on or about February 8, 2016, employees were performing connecting work, approximately 16 feet above a lower level, while wearing personal fall arrest equipment which did not include the necessary components to effectively tie off to the structure being assembled.

29 CFR 1926.1425(c)(2): The employer did not make sure that hooks with self-closing latches were used while employees were hooking, unhooking, or guiding the load. a) Main street past swamp Road, Doylestown, Worksite - on or about February 8, 2016, employees were lifting steel beams to height while using a crane hook that utilized a self-closing latch that was not properly functioning.
